is island paradise available to download?

Island Paradise doesn't even come out until tomorrow. The prepatch came out the other day, but is only just now available on ZLOrigin. Patience.. ZL is extremely slow at the moment.

Is the only way to get the update is with ZLOrigin? I have all of the expansions already downloaded but I don't have ZL

Not the only way by any means. You can also wait until is surfaces here.

I'd wait until it surfaces here, playing the G4TW version of TS4 is currently my favourite version/way to play.

I'm noticing a lot of people (online) waiting for this new patch.

While I won't/can't give an ETA, don't expect it *now*. It's now 2AM in the Central European Time, and for the people there I'd suggest you get some sleep. Check back later in the day...
