If I have a .sim file and no SavedSims folder...
can I just make a new one? Or should I just save a decoy sim and then install my downloaded sims into the created folder afterwards?

Sorry for the late reply, but yes, if you don't have that folder, make it yourself. The folder's name must be (exactly) SavedSims. That folder must be in My Documents/Electronic Arts/The Sims 3.
Put your .sim file into that SavedSims folder, and in-game, look either under the button "premade sims" in CAS (that's in the lower left part of the screen, in CAS; and it's a relatively big button). Or you can also look in "edit town", under households for that sim.
