GTW: Issues with doctor career

I have looked everywhere for an answer to this issue and have been unable to find even one other person having this issue. I just made one of my sims a doctor. When its time to go to work if I let her go to work and I don't "Follow her" she goes to work and everything is great BUT when I go with her to work then instead of going to work she stays at home her work starts meaning I get the timer in my upper left corner with all the things it wants me to accomplish and patients start showing up at my house!! I cannot treat them or examine them since I am not at the hospital. I have tried this now with 2 different sims in 2 different households so its an all around issue.
Also when one of my sims had a baby and I chose the have baby at hospital option she would never go to the hospital. I kept trying to send her and send her but she would not go and eventually had the baby at home.
Its almost like the hospital is not actually there or something.....I'm going out of my mind trying to fix this please help!!!!!!
